- Insurance claim forms
- Prescriptions for taking medication abroad
- Private sick notes
- Vaccination certificates
These services are only available outside of normal appointment times.
There is a charge for all non-NHS services. Please be prepared to pay at the time you receive the service. This is a list of the most used services only. Further information on other services can be given on request.
